Idaho Falls School District 91 prides itself on having effective, high quality teachers in its classrooms. Parent input now is considered as part of our teachers' annual performance evaluations. 

Parents are encouraged to provide input. It's easy. Just follow these steps.  

  1. ​​Download a Parent Input Form or pick one up in the school office.
  2. Complete the questionnaire, sign it, place it in a sealed envelop and return it to the school office or mail it to the school.
  3. Only signed forms will be considered.
  4. Parents can only complete one form per teacher per year.
